Terms & Conditions*
- I am adopting the pet described below through Pooch Carriage Crew. The pet is being adopted as a companion and will live as a family member with me inside my home. I will not allow the pet to be used for purposes of vivisection or experimentation.
- I will provide proper and sufficient, food, water, indoor shelter and kind treatment for the pet at all times. I further agree the dog will not be chained at any time as a means of restraint or be a primarily outside dog.
- I agree that a representative of Pooch Carriage Crew may make periodic inspections of the pet, my residence and the pet’s living conditions on reasonable notice. I further agree to surrender the pet to Pooch Carriage Crew’s representative if for any reason he/she considers the conditions provided by me for the pet to be unsatisfactory.
- I will keep a collar and identification tag on the dog at all times and will license the dog with the county/city. I will maintain current information to ensure I am able to be contacted in the event my dog escapes my residence or care.
- I agree to immediately notify Pooch Carriage Crew if the pet is lost or stolen. I further agree, in the event the animal is lost, that I will make every reasonable effort to attempt to locate and claim the animal, and to immediately retrieve the pet from any public pound or animal shelter when notified the animal is being held at any such location and to pay any costs associated with its recovery.
- Pooch Carriage Crew has provided me (or will provide me upon arrival of the dog) with the following: Adoption Contract + Health Records
- I will maintain my pet's health with routine vetrinary visits, heartworm prevention, and flea/tick prevention. If I am unable or unwilling to provide routine health care I will contact the rescue for additional resources, or I will return the dog to Pooch Carriage Crew.
